8. Letter from employer confirming period of approved paid/unpaid leave of absence from place of work (if the applicant is still working
9. Authorization of leave from School for children
10. Salary slips for the last three months and Documentary proof of EPF contribution Pension document (if not working anymore)
11. Company/Personal Bank Statements for the last three months
12. Business registration and form 64
13. Any other proof of financial soundness
14. Birth and marriage certificates+ English translation

Business

8. Letter explaining who is paying for the trip
9. Invitation or proof of business contact/seminar/workshop (invitation letter) in the Schengen state, indicating purpose and duration of the visit/mission. Caution : As part of professional assignments even for short mission, a temporary work permit shall be applied to the relevant Administration. Please check with the business counterpart and undertake the necessary steps to fulfil them at all times and on time (e.g.: in France, it may take several weeks to obtain an “autorisationprovisoire de travail” from the DIRECCTE).
10. Letter from company / employer in Sri Lanka indicating in detail purpose and duration of visit/mission
11. Salary slips for the last three months and Documentary proof of EPF contribution
12. Company / Personal Bank Statements for the last three months (original + copy). + Overdraft facility (if debit).
13. Business registration and form 64 (companies act n°17 of 1982) both withEnglish translation (original + copy)

Airport Transit

1. Valid passport
2. Required visa for the final destination or official proof that a visa on arrival will be granted to the applicant
3. Application form dated and signed
4. Photograph
5. Confirmed itinerary
6. Travel Insurance or Policy details if mentioned in the shipping company letter - valid for the length of the stay with extra 15 days and for all the Schengen states, covering medical repatriation, emergency medical and hospital care - A minimum cover of 30,000 euros is mandatory
7. Letter from Ministry of Ports and Shipping with details of applicant and date/place of joining the ship
8. Letter from local shipping agent (insurance cover must be mentioned in the said letter)
9. Certificate of Discharge
10. Letter from foreign shipping agent
11. Seaman’s book (all pages)

6. Accommodation Certificate issued by the local Municipal Council to sponsor (original + copy) along with appropriate pertaining documents from the sponsor

9. If the sponsor is not a French national: copy of his/her passport and residential status
10. Salary slips for the last three months
11. Bank Statements for the last three months
